1. Website owner and contact
This website is a professional information and patient-education profile for Dr. Nishat Anam Borna.
Data controller / responsible site owner: To be confirmed by the site owner before launch.
Privacy contact: Add a verified professional email or postal contact before launch.
Do not publish this page as final until the responsible person or organisation and a working privacy contact have been confirmed.
2. Information that may be involved
The current project is a static front-end prototype. The inquiry form and appointment walkthrough generate a preview in the visitor's browser only. This package does not connect either form to a clinic, email service, booking platform, database, or external form processor.
If a real appointment inquiry service is added later, it may involve details such as:
- Name and preferred contact details.
- General inquiry category and preferred appointment date.
- An optional short message supplied by the visitor.
- Technical information processed by the hosting service, such as request time, IP address, device or browser information, and security logs, depending on the provider's configuration.
Please do not enter detailed medical histories, identity numbers, financial information, or other sensitive information into the current demo form. The site must not request more data than is necessary for a defined and lawful purpose.
3. Purposes and legal basis
If an actual inquiry or booking integration is enabled, information should be used only for a clearly stated purpose, such as responding to an appointment question or managing the visitor's request. The site owner must determine and document the applicable legal basis under the law that governs the final service, and provide any required notice or consent.
The current prototype does not process inquiry form details through a backend. This statement must be updated if that behaviour changes, including when a third-party messaging or scheduling service is connected.
5. Data retention and security
The demo page does not save form input to its own database or browser storage. Information typed into a demo form remains temporarily in the open page and can be removed by reloading or closing it; the page does not submit it to a practice.
For a future live service, the site owner must set a proportionate retention period, restrict staff access, use appropriate security controls, and explain how appointment inquiries are deleted. No website can promise absolute security, and the actual controls depend on the hosting and integration configuration.
6. Your choices and rights
Do not use the demo form to send a real appointment request. For appointment inquiries, call the listed chamber telephone number on the main page and confirm availability directly. A separate privacy contact for data requests still needs to be designated by the site owner.
Depending on the applicable law and the final processing setup, people may have rights to request access to, correction of, deletion of, or information about their personal data, and to raise concerns or object to certain processing. Ask the site owner to confirm the rights and process that apply in the relevant jurisdiction.
How to submit a privacy request: Add the verified privacy contact before launch.
8. Policy updates
This draft was prepared on 19 October 2026 for review. Update the policy when the website's collection practices, hosting, contact channels, or integrations change. Replace the draft effective date with the date the site owner approves and publishes the final version.
